This infobox template is used for local affiliates.

Parameters

Name

Name of the organization

Logo

File name of logo (example: LPNY.png)

Jurisdiction

The area in which the affiliate covers, such as a particular county, group of counties, parish, borough, city, town, etc.

State

The state in which the affiliate is located.

Established

Date of establishment, such as an inaugural meeting

Chartered

Date of charter (such as approval from state)

Decharted

Date of decharted for defunct affiliates.

Status

Use "Active" or "Defunct"

Chair, Vice Chair, Secretary, Treasurer

Name of office holder

Website, Facebook, Twitter, Meetup

URL of page without the "[" and "]"

Usage

Copy and paste the following to the top of the page you want to insert the template and fill out the information.
